Here’s a short tips on how to apply for and get an SSS ID:<br />

1. Download the E-6 form from the SSS website. Click on Forms at the left navigation pane. Then select E-6 from the list. Or click here for the direct link: E6 Form. Print then fill up the information needed.<br />

2. Click on Employment History at the left panel. You will be shown a list of all your employers, past and present. Copy the employer number of your company into the form.<br />
<br />
3. Click SSS ID Clearance at the left panel. If the note says “Member Qualified for an SSS ID Card”, then you are cleared and eligible to apply for the ID. Print this page. Just click the File menu of the browser, then Print.<br />
<br />
4. Go to the SSS office. Tell the guard you already have an SSS number, the E-6 form and the ID Clearance and that you are applying for the ID. He should give you a priority for your picture to be taken.<br />
<br />
5. Get in line at the SSS ID section and get your picture taken. Present two IDs from the list stated at the 2nd page of the E-6 form. If you have your company ID and an ATM card with your complete name, then you should be fine.<br />
<br />
6. You’re done. Just wait for the SSS ID to be mailed at your home or office address. The objective of the new procedure, the SSS said, is to ensure the accuracy and validity of basic member information to minimize, if not eliminate, incidents of failed registration and/or non-receipt of e-mail on registration outcome.<br />

SSS Online Registration Process as of July 2011:<br />
<br />
<b>PHASE 1</b><br />
You will be required to provide the following information: SS Number, First Name, Middle Name, Last Name, Date of Birth and Email Address.<br />
<br />
Upon submission of the information, these will be checked with your SSS records. If correct, the system will display the message that an e-mail has been sent to your e-mail address. The said e-mail will advise you to click on the given link to continue with your registration.<br />
<br />
In case, however, that the information supplied is different from SSS records, appropriate error message/s will be displayed on-screen to inform and guide the registrant.<br />
<br />
<b>PHASE 2</b><br />
You will be asked to provide additional personal information such as: Mother's maiden name, Address, Contact Number, Preferred User ID and Password, Current Membership Status, and Employer ID Number (for employed member or household help only) or Receipt No./Over-the-Counter (OTC) transaction number (for OFW, Voluntary and Self-employed members).<br />
If your registration is successful, you will receive an e-mail containing your User ID and Password. If not, the e-mail will inform you that you have not successfully registered in the SSS website and advise you to call (632) 9206401, local 6201 or e-mail member_assistance@sss.gov.ph for assistance.<br />
The SSS said that a member may also seek the help of the nearest branch office.<br />
